Referral and Admission Procedure
To refer yourself or someone else into treatment there is nothing simpler just contact our outreach department.
When contact is made with Ark House regarding a potential admission, an evaluation and assessment is carried out. The assessment establishes the need of care required as well as establishing if Ark House can meet that need with its staff and resources. Any potential risks will also be identified at this stage. The assessment is also an opportunity for the service user and their family to ask any questions that they may have regarding the service that we offer.
The following information will be requested; Name, address, telephone number, date of birth, sex, National Insurance Number, sources of referral, present problems, drug or alcohol abuse history, psychological and physical health history, current and past medication and any relevant information which will assist in formulating an initial assessment of the service user such as past criminal history.

Restrictions
- The service user must be 18 years or older.
- He or she must be physically and mentally capable of absorbing and participating in regular activities of the residential facility.
- He or she must not be on any prescribed medications that is on our prescribed list (Please enquire).
