Referral and Assessment Procedure
Once you contact Ark House regarding a potential admission, our team will carry out a comprehensive evaluation and assessment. This process helps us determine:
• The level and type of care required
• Whether Ark House has the appropriate staff, facilities, and resources to meet your needs
• Any potential risks that may need to be managed
The assessment is also an opportunity for both the prospective resident and their family to ask questions about our services, programmes, and recovery approach.
Information typically requested during assessment includes:
• Date of birth and NHS number
• Gender
• Source of referral
• Current drug or alcohol use and history
• Psychological and physical health history
• Current and past medication
• Relevant background information, including any criminal history

Referral Requirements
• Be 18 years or older
• Be physically and mentally capable of participating in regular activities at the residential facility
